Assessments and Care Planning

In order to determine the appropriate care package for you and identify your needs, our care team will carry out an assessment of needs which can be completed at your home at a time that suits you.

We will go through various questions to determine your needs to produce a personalised and outcome focused Care/ Support Plan and Risk Assessments. These will be developed with your input, your family or representatives to identify how your needs will be met and setting out the outcomes to be achieved.

Once agreed we can arrange when care will commence including the calls times and care tasks that our staff will assist you with.

If and when your needs change, we will review your Care/Support Plan and Risk Assessments to ensure that the service we are providing is adapted to your needs at all times.

Domiciliary Care

This is the general term for care provided in a client's own home. It encompasses a wide range of services, including personal care, companionship, meal preparation, and housekeeping.

Companionship Care

We provide social interaction, emotional support, and companionship to clients who may feel isolated or lonely. Companionship care can include activities like conversation, playing games, going for walks, or simply being there for a listening ear.

End of Life Care

this service provides comfort and support to clients nearing the end of their life. Our caregivers can assist with pain management, emotional support, and practical tasks to ensure a dignified and peaceful end-of-life experience for both the client and their family.

Live-in Care

We provide continuous care for clients who require 24/7 support. A qualified caregiver will live in the client's home and be available to assist with all their needs, including personal care, medication management, meal preparation, and housekeeping.

Overnight Care

This is support during the nighttime hours, ensuring the client's safety and well-being while they sleep. Overnight caregivers can assist with toileting needs, medication reminders, and provide companionship for those who may feel anxious at night.

Home from Hospital Care

This service provides support to clients who are transitioning back home after a hospital stay. Our caregivers can assist with medication management, wound care, following discharge instructions, and ensuring a smooth and safe recovery at home.

Safegurding

We have a robust Safeguarding policy which reflects that duties apply under the Care Act 2014.  The Care and support statutory guidance states that ‘all workers need to be vigilant about adult safeguarding concerns and that safeguarding practice should be underpinned by the six safeguarding principles:
